Madhya Pradesh NEET MBBS Counselling Documents

Every document a Madhya Pradesh candidate needs, covering the national papers every applicant must carry, plus the DME state-quota proofs and bonds. What each one is, how to get it, what it costs, and the mistakes that cost a seat.

Madhya Pradesh fences early rounds for locals and binds you with a heavy bond

You qualify for the MP state quota by being a permanent resident (mool nivasi) of Madhya Pradesh or by having passed both Class 10 and Class 12 from recognised MP schools. The state reserves its government and private state-quota seats for MP candidates in the early rounds. After allotment you execute bonds on stamp paper, including a seat-leaving bond reported at Rs.30 lakh for government colleges.

Two things to plan for in Madhya Pradesh

Domicile or 10+12 in MP. A Madhya Pradesh permanent-resident (mool nivasi) certificate, or Class 10 and 12 passed from MP schools, is required for the state quota.

The bonds. After admission to a government college you sign a seat-leaving bond (reported at Rs.30 lakh) and a rural-service bond on non-judicial stamp paper. Confirm the current bond amounts and proformas in the 2026 rule book.

You are considered for the 85% state quota and the 15% All India Quota run by the MCC. The documents below cover both. One difference for AIQ: your OBC certificate must be on the Central OBC-NCL list in the central format, because an MP OBC certificate works for the state quota but is rejected for AIQ. If you plan to use both, get a central-list OBC-NCL certificate as well.

Class 10 Certificate / Marksheet

Recognised national or state board

What it is
The accepted statutory proof of date of birth. The board certificate, not a municipal birth certificate, is what the counselling authority wants.
How to get it
You already have it. If lost, apply to your board for a duplicate well before counselling.
What it costs
Free if you have it; duplicate ~Rs.100–500 from the board.

Instant if held; a board duplicate can take 2–4 weeks.

Class 12 Marksheet + Passing Certificate

Recognised national or state board

What it is
Proof you cleared 10+2 with Physics, Chemistry, Biology/Biotech and English, and met the aggregate (50% Open/EWS, 40% SC/ST/OBC, 45% Open-PwBD).
How to get it
You already have it. Must show individual subject marks.

Government Photo ID

Aadhaar / PAN / Passport / Voter ID / Driving Licence

What it is
Identity check against the admit card to stop impersonation.
How to get it
Use any valid, unexpired government ID. Aadhaar is simplest as the name must match across documents.

Passport-size Photographs

Same photo as your NTA registration

What it is
For college records. Must be identical to the photograph uploaded during NEET registration.
How to get it
Get 8–12 colour copies printed from the exact file you uploaded to NTA.

~Rs.50–150 at a photo studio.

Medical Fitness Certificate

Registered allopathic practitioner (MBBS or higher)

Standard
What it is
Confirms you are fit for clinical training. Some colleges issue it at reporting; others want it in advance.
How to get it
Get it from a registered MBBS doctor, or at the allotted college's own medical board if they insist on theirs.
What it costs
Free at a government hospital; Rs.200–1,000 privately.
How long
Same day.

Birth certificate: only needed as a backup if your Class 10 certificate does not print the exact date of birth.

Seat-Leaving & Rural-Service Bonds

Commitment bonds: a seat-leaving bond (reported at Rs.30 lakh for government colleges) and a rural-service bond.
How to get it
Execute the bonds on non-judicial stamp paper in the DME proformas after seat allotment.

Watch out: Bond amounts and proformas change, so verify the current figures in the 2026 rule book before reporting.

What it is
Proof of your reserved category for the MP quota. MP gives reserved categories an age relaxation, so accurate DOB on your Class 10 certificate matters.

Apply on the MP e-District portal in the prescribed format. EWS is for the current year.

Watch out: OBC and EWS certificates are time-limited, so get freshly dated ones for this cycle.

PwBD Certificate

State-specified authority / MCC centre (AIQ)

How to get it
For the state quota, use the route in the DME rule book with a valid UDID card. For AIQ you need a separate certificate from one of the MCC's roughly 16 designated centres.

Watch out: A state PwBD certificate does not cover your AIQ PwBD claim, and vice versa.

The Madhya Pradesh checklist
