Madhya Pradesh NEET MBBS Counselling Documents
Every document you need as a Madhya Pradesh candidate. It covers the national papers that everyone has to carry, plus the DME state-quota proofs and bonds. For each one you get what it is, how to get it, what it costs, and the mistakes that can cost you a seat.
Madhya Pradesh keeps early rounds for local candidates and ties you to a large bond
You qualify for the MP state quota in one of two ways: by being a permanent resident (mool nivasi) of Madhya Pradesh, or by having passed both Class 10 and Class 12 from recognised MP schools. In the early rounds the state keeps its government and private state-quota seats for MP candidates. Once you are allotted a seat, you sign bonds on stamp paper, including a seat-leaving bond reported at Rs.30 lakh for government colleges.
Two things to plan for in Madhya Pradesh
Domicile or 10+12 in MP. For the state quota you need a Madhya Pradesh permanent-resident (mool nivasi) certificate, or proof that you passed Class 10 and 12 from MP schools.
The bonds. After you get into a government college you sign a seat-leaving bond (reported at Rs.30 lakh) and a rural-service bond on non-judicial stamp paper. Check the current bond amounts and proformas in the 2026 rule book.
You are considered for the 85% state quota and the 15% All India Quota run by the MCC. The documents below cover both. One difference for AIQ: your OBC certificate must be on the Central OBC-NCL list in the central format, because an MP OBC certificate works for the state quota but is rejected for AIQ. If you plan to use both, get a central-list OBC-NCL certificate as well.

Documents every candidate needs

NEET UG 2026 Admit Card
National Testing Agency (NTA)
- What it is
- This is your proof that you sat the exam. It carries your roll number and the photo used to match your face at the desk.
- How to get it
- Download it from neet.nta.nic.in. A fresh admit card is being issued for the 21 June 2026 re-exam, so your old 3 May admit card no longer counts.

- Free.

- You can download it the moment it is released, expected by mid-June 2026.
NEET UG 2026 Scorecard / Rank Letter
National Testing Agency (NTA)
- What it is
- This shows your percentile, your marks and your All India Rank. The whole allotment runs on this rank.
- How to get it
- Download it from neet.nta.nic.in once results are out, expected July 2026.
- What it costs
- Free.
- How long
- You can download it the moment results are declared.
Class 10 Certificate / Marksheet
Recognised national or state board
- What it is
- This is the accepted proof of your date of birth. The counselling authority wants the board certificate, not a municipal birth certificate.
- How to get it
- You already have this one. If you have lost it, apply to your board for a duplicate well before counselling starts.
- What it costs
- Free if you have it; a duplicate from the board costs about Rs.100–500.
- How long
- Ready straight away if you have it; a board duplicate can take 2–4 weeks.
Class 12 Marksheet + Passing Certificate
Recognised national or state board
- What it is
- This proves you cleared 10+2 with Physics, Chemistry, Biology/Biotech and English, and met the required aggregate (50% Open/EWS, 40% SC/ST/OBC, 40% Open-PwBD).
- How to get it
- You already have this one. It must show your individual subject marks.
- What it costs
- Free if you have it.
- How long
- Ready straight away if you have it.
Government Photo ID
Aadhaar / PAN / Passport / Voter ID / Driving Licence
- What it is
- This checks your identity against the admit card so nobody can sit in your place.
- How to get it
- Bring any valid, unexpired government ID. Aadhaar is the simplest choice, since your name has to match across all your documents.
- What it costs
- Free if you have it.
- How long
- Ready straight away if you have it.
Passport-size Photographs
Same photo as your NTA registration
- What it is
- These are for the college's records. They must be the same photo you uploaded during NEET registration.
- How to get it
- Print 8 colour copies from the exact file you uploaded to NTA.
- What it costs
- About Rs.50–150 at a photo studio.
- How long
- Same day.
Medical Fitness Certificate
Registered allopathic practitioner (MBBS or higher)
- What it is
- This confirms you are fit for clinical training. Some colleges issue it when you report; others want it in hand beforehand.
- How to get it
- Get it from a registered MBBS doctor, or from the allotted college's own medical board if they insist on using theirs.
- What it costs
- Free at a government hospital; Rs.200–1,000 at a private one.
- How long
- Same day.
Birth certificate: only needed as a backup if your Class 10 certificate does not print the exact date of birth.
Madhya Pradesh eligibility
These open up the 85% state quota.
Domicile Certificate
District Magistrate / competent revenue authority (MP e-District)
- What it is
- Proof that you live in Madhya Pradesh, which is what makes you eligible for the state quota. It is item 14 on the DME admission checklist for every state-quota candidate.
- How to get it
- Apply on the MP e-District portal to the competent revenue authority. The checklist treats it as a required document in its own right, not as something your MP Class 10 and 12 certificates can replace.
- What it costs
- Nominal.
- How long
- Allow about 2 weeks; apply early.
Watch out: Bring the original plus 4 self-attested photocopies, in the checklist order, for scrutiny. The state quota also needs the separate Proforma-7 domicile affidavit (below); the certificate on its own is not enough.
Rural-Service & Seat-Leaving Bond Affidavits (Proforma-8)
Candidate (notarised affidavit, executed after allotment)
- What it is
- Two bonds on non-judicial stamp paper: the Rural-Service Bond (Proforma-8(v&1)/8(v&2)) and the Seat-Leaving Bond (Proforma-8(c)). The seat-leaving bond sets the government-college figure at Rs.30 lakh.
- How to get it
- Sign both bonds in the DME proformas after you are allotted a seat. For your first admission you make the bonds on the proforma you carry at reporting. If you take an upgrade in a later round, you re-submit the rural-service and seat-leaving affidavits on a Rs.500 non-judicial stamp as per the proforma on the college website.

Watch out: These are required checklist items (22 and 23), not optional. The seat-leaving amount and proformas can change, so check the current figures in the 2026 DME rule book before you report.
Proforma-7 Domicile Affidavit
Candidate (notarised affidavit on Rs.100 non-judicial stamp paper)
- What it is
- A sworn affidavit stating that you are a permanent resident of MP. The state quota needs this on top of the Domicile Certificate.
- How to get it
- Make Proforma-7 on a Rs.100 non-judicial stamp paper and get it notarised (checklist item 21, marked "for state quota counselling only"). If a parent or guardian makes it, the guardian signs.
- What it costs
- Stamp-paper / notary cost.
- How long
- Same day to a few days.
Watch out: This is separate from the Domicile Certificate; the state quota needs both.
MP Online Registration Receipt
MP Online (DME state-quota counselling portal)
- What it is
- The receipt that confirms you registered for MP DME state-quota counselling. It is item 4 on the checklist, marked "for state quota counselling only".
- How to get it
- Register on the DME MP Online portal (dme.mponline.gov.in) and print the registration receipt.
- What it costs
- Registration / portal fee.
- How long
- Available immediately after you register.
Watch out: Needed only for state-quota counselling; for AIQ/MCC you carry the MCC allotment letter instead.
Reservation & fee documents
Only if it applies to youOnly if you are claiming a reserved category.
Caste Certificate (SC / ST / OBC / EWS)
Competent authority (the EWS income-and-assets certificate is issued by the Tehsildar)
- What it is
- Proof of your reserved category for the MP state quota, covering SC, ST, OBC and EWS. EWS in MP is the 10% reservation for the economically weaker section among candidates not covered by SC/ST/OBC, with the family income limit set at Rs.8 lakh.
- How to get it
- Apply to the competent authority in the prescribed format (checklist item 15). For EWS, the Tehsildar issues the income-and-assets certificate; the Income Certificate of the current financial year (item 16) is a separate required document.
- What it costs
- Nominal.
- How long
- The EWS income-and-assets certificate takes about 15 working days; allow 2 weeks for the others.
Watch out: OBC and EWS certificates are time-limited, so get freshly dated ones for this cycle. SC/ST certificates do not expire each year.
Income Certificate (current financial year)
Tehsildar / competent revenue authority
- What it is
- An income certificate of the current financial year, needed where it applies (for example for EWS and other income-linked claims).
- How to get it
- Apply on the MP e-District portal to the Tehsildar for an income certificate dated in the current financial year (checklist item 16).
- What it costs
- Nominal.
- How long
- About 2 weeks.
Watch out: It must be of the current financial year; an older income certificate is rejected.
Special quotas

Government-School 5% Quota Certificate (Proforma-10)
District Education Officer; District Coordinator / Assistant Commissioner
- What it is
- The certificate for the MP government-school 5% quota, for candidates who studied Class 6 to Class 12 in recognised MP government schools.
- How to get it
- Get Proforma-10 (Certificate for Govt. School Students), signed by the District Education Officer and the District Coordinator / Assistant Commissioner, and carry it with your original mark sheets (checklist item 17).
- What it costs
- Nominal.
- How long
- Apply early.
Watch out: Needed only for the government-school quota, and only with the original Class mark sheets that prove Class 6 to 12 in MP government schools.
Person with Disability Certificate + UDID Card
Competent authority on the DGHS/NMC-notified format (state quota); MCC designated centre (AIQ)
- What it is
- For the Persons with Disability horizontal quota. The MP checklist asks for both the disability certificate and the UDID card.
- How to get it
- For the state quota, get the disability certificate from the competent authority on the format notified under MCI/NMC and DGHS, and carry your UDID card (checklist item 18). For AIQ you need a separate certificate from one of the MCC's designated centres.

- Free at a government facility.
- How long
- Apply early; slots are limited.
Watch out: You need both the certificate and the UDID card for the state quota. A state PwD certificate does not cover your AIQ claim, and an AIQ one does not cover the state quota.
Military Personnel / Ex-Servicemen Certificate (Proforma-2)
District Sainik Welfare Officer
- What it is
- The certificate that backs a claim under the military / ex-servicemen (SN) class. MP uses Proforma-2(a) for an ex-serviceman, Proforma-2(b) for one serving in or outside MP, and Proforma-2(c) for one permanently residing in MP.
- How to get it
- Get the right Proforma-2 variant signed by the District Sainik Welfare Officer (checklist item 19).
- What it costs
- Nominal.
- How long
- Apply early.
Watch out: Use the correct Proforma-2 sub-form for your case; the same checklist item also covers the freedom-fighter claim under Proforma-3.
Freedom-Fighter Certificate (Proforma-3)
District Collector (or the authorised officer)
- What it is
- The certificate that backs a claim under the freedom-fighter (FF) class, for the ward or descendant of a freedom fighter registered in MP.
- How to get it
- Get Proforma-3 (Freedom-Fighter category certificate) signed by the District Collector or the authorised officer, with the office seal (checklist item 19).
- What it costs
- Nominal.

- Apply early.

Every candidate carries these at reporting, in the checklist order, with the original plus 4 self-attested photocopies.
Class 11 Mark Sheet
Recognised board / school
- What it is
- Your Class 11 mark sheet, listed as a required document on the MP DME checklist (item 8).
- How to get it
- You already have it; keep the original ready.
- What it costs
- Free if held.
- How long
- Instant if held.
Watch out: MP asks for the Class 11 mark sheet on top of the Class 10 and Class 12 documents in the core list.
Character Certificate
Head of the last institution attended
- What it is
- A character certificate from the school or college you last attended (checklist item 11).
- How to get it
- Ask the head of your last institution for it.
- What it costs
- Nominal.
- How long
- A few days.
Watch out: Bring the original plus self-attested photocopies.
Transfer & Migration Certificates
Last school / board attended
- What it is
- The Transfer Certificate (item 12) and Migration Certificate (item 13) from your last institution / board.
- How to get it
- Ask your school for the Transfer Certificate and your board for the Migration Certificate.
- What it costs
- Nominal board fee.
- How long
- A few days to a few weeks from the board.
Watch out: The MP checklist asks for both; apply for the Migration Certificate early as boards can be slow.
Gap Affidavit
Candidate (notarised affidavit), if applicable
- What it is
- An affidavit explaining any gap in your education, needed only if there is a break between qualifying exams (checklist item 20).
- How to get it
- Make a notarised gap affidavit explaining the period and reason for the break.
- What it costs
- Stamp-paper / notary cost.
- How long
- Same day to a few days.
Watch out: Needed only if you have an education gap; carry it to reporting if so.
Proforma-9 (Anti-Ragging & Conduct Affidavit)
Candidate (notarised affidavit on Rs.100 non-judicial stamp paper)
- What it is
- An affidavit on stipend, strike, ragging, and professional etiquette and ethics (checklist item 24), made under the medical-council regulations.
- How to get it
- Make Proforma-9 on a Rs.100 non-judicial stamp paper and get it notarised.
- What it costs
- Stamp-paper / notary cost.
- How long
- Same day to a few days.
Watch out: Every admitted candidate has to give this, not just reserved categories.
The Madhya Pradesh checklist
- Eligibility: MP mool nivasi certificate, or Class 10 and 12 passed from MP.
- Bonds: get ready for the seat-leaving and rural-service bonds on stamp paper after allotment.
- Category: current caste/EWS certificates; remember the reserved-category age relaxation.
