Assam NEET MBBS Counselling Documents

Every document you need as an Assam candidate, covering the national papers every applicant carries plus the DME state-quota proofs. For each one: what it is, how to get it, what it costs, and the mistakes that cost a seat.

Assam holds its seats with the PRC, and a special EWS annexure

The Permanent Resident Certificate (PRC) is the backbone of an Assam claim, proving you have lived in the state long-term. The thing to watch is EWS: Assam will not take a generic EWS certificate. Your claim has to be on the state's Annexure-XI, signed off specifically by a Circle Officer.

Two Assam-specific items

Permanent Resident Certificate (PRC). This is the main proof that you live in Assam for the state quota, issued by the competent Assam authority.

EWS on Annexure-XI. Your EWS claim has to be on Assam's Annexure-XI, signed by a Circle Officer. A generic EWS certificate will not do.

You are considered for the 85% state quota and the 15% All India Quota run by the MCC. The documents below cover both. One difference for AIQ: your OBC certificate must be on the Central OBC-NCL list in the central format, because an Assam OBC/MOBC certificate works for the state quota but is rejected for AIQ. If you plan to use both, get a central-list OBC-NCL certificate as well.

NEET UG 2026 Admit Card

National Testing Agency (NTA)

Standard
What it is
This is your proof that you sat the exam. It carries your roll number and the photo used to match your face at the desk.
How to get it
Download it from neet.nta.nic.in. A fresh admit card is being issued for the 21 June 2026 re-exam, so your old 3 May admit card no longer counts.

Free.
How long
You can download it the moment it is released, expected by mid-June 2026.

NEET UG 2026 Scorecard / Rank Letter

National Testing Agency (NTA)

Standard
What it is
This shows your percentile, your marks and your All India Rank. The whole allotment runs on this rank.
How to get it
Download it from neet.nta.nic.in once results are out, expected July 2026.
What it costs
Free.
How long
You can download it the moment results are declared.

Class 10 Certificate / Marksheet

Recognised national or state board

Standard
What it is
This is the accepted proof of your date of birth. The counselling authority wants the board certificate, not a municipal birth certificate.
How to get it
You already have this one. If you have lost it, apply to your board for a duplicate well before counselling starts.
What it costs
Free if you have it; a duplicate from the board costs about Rs.100–500.
How long
Ready straight away if you have it; a board duplicate can take 2–4 weeks.

Class 12 Marksheet + Passing Certificate

Recognised national or state board

Standard
What it is
This proves you cleared 10+2 with Physics, Chemistry, Biology/Biotech and English, and met the required aggregate (50% Open/EWS, 40% SC/ST/OBC, 40% Open-PwBD).
How to get it
You already have this one. It must show your individual subject marks.
What it costs
Free if you have it.
How long
Ready straight away if you have it.

Government Photo ID

Aadhaar / PAN / Passport / Voter ID / Driving Licence

Standard
What it is
This checks your identity against the admit card so nobody can sit in your place.
How to get it
Bring any valid, unexpired government ID. Aadhaar is the simplest choice, since your name has to match across all your documents.
What it costs
Free if you have it.
How long
Ready straight away if you have it.

Passport-size Photographs

Same photo as your NTA registration

Standard
What it is
These are for the college's records. They must be the same photo you uploaded during NEET registration.
How to get it
Print 8 colour copies from the exact file you uploaded to NTA.
What it costs
About Rs.50–150 at a photo studio.
How long
Same day.

Medical Fitness Certificate

Registered allopathic practitioner (MBBS or higher)

Standard
What it is
This confirms you are fit for clinical training. Some colleges issue it when you report; others want it in hand beforehand.
How to get it
Get it from a registered MBBS doctor, or from the allotted college's own medical board if they insist on using theirs.
What it costs
Free at a government hospital; Rs.200–1,000 at a private one.
How long
Same day.

Birth certificate: only needed as a backup if your Class 10 certificate does not print the exact date of birth.

Assam eligibility

These open up the 85% state quota.

Permanent Resident Certificate (PRC) — Annexure-I

Deputy Commissioner (or authorised officer) of the concerned district

What it is
The main proof that you live in Assam, for the state quota. The gazette format certifies 20 years of continuous residency in Assam for you or for your father/mother.
How to get it
Get the PRC in the gazette's Annexure-I format, signed by the Deputy Commissioner or his/her authorised officer in your district. You can apply through the Assam e-District / Sewa Setu portal, but for counselling the certificate itself has to carry the DC's sign-off.
What it costs
Nominal.
How long
Allow about 2 weeks; apply early.

Watch out: It has to be the Annexure-I PRC certifying 20 years of continuous residency, signed by the DC of the concerned district. Sons/daughters of All India Service officers posted to Assam are exempt from the 20-year rule, but instead they need a service certificate of the father/mother from the concerned Assam Government authority/department.

Certificate of Study in Assam (Class VII to XII) — Annexure-II

Head Master / Principal of each school attended

What it is
Proof that you studied every class from Class VII to XII in Assam and passed the qualifying exam from an institute in Assam. This is a core state-quota eligibility condition alongside the PRC.
How to get it
Get the Annexure-II study certificate signed and sealed by the Head Master/Principal of each school you attended. If you studied at more than one school, you need a separate certificate from each.
What it costs
Nominal / free from the school.
How long
Allow 1-2 weeks to collect from each school.

Watch out: If you studied outside Assam from Class VII onwards because your father/mother was posted outside the state as an Assam Government, Central Government or government corporation/agency employee, that period can be relaxed. But you have to show a certificate of the parent's employment outside the state with the period of service.

Reservation documents

Only if it applies to you

Only if you are claiming a reserved category.

Caste Certificate (SC / ST(P) / ST(H) / OBC-MOBC)

Deputy Commissioner (DC) of the concerned district

Time-bound
What it is
Proof of your reserved category for the Assam state quota. For OBC/MOBC the certificate also has to state your Non-Creamy-Layer (NCL) status; SC, ST(Plain) and ST(Hills) need only the caste certificate.
How to get it
Apply for the original caste certificate from the concerned Deputy Commissioner (DC). At counselling you submit it with the matching gazette annexure: OBC/MOBC on Annexure-III (NCL), SC on Annexure-IV, and ST(P)/ST(H) on Annexure-V. The OBC/MOBC quota covers the TGL/Ex-TGL (Annexure-VI), Moran, Motak, Tai Ahom, Chutiya and Koch Rajbongshi communities, each certified as OBC-NCL by the DC.

About 2 weeks; apply early.

Watch out: OBC/MOBC certificates have to state Non-Creamy-Layer status. If you are in the creamy layer, or your community is not on the Assam OBC/MOBC list, apply as General. The certificate has to come from the DC, not a lower revenue officer.

EWS Certificate (Circle Officer)

Concerned Circle Office / Circle Officer of Assam

What it is
For the 10% Economically Weaker Section quota in the Assam state pool (10% of the total state-quota seats).
How to get it
Get the EWS certificate from the concerned Circle Office or Circle Officer, issued as per Office Memorandum No. ABP 07/2019/4 dated 10 April 2019 of the Personnel Department, Government of Assam.
What it costs
Nominal.
How long
About 2 weeks.

Watch out: It has to be the Assam EWS certificate from the Circle Office/Circle Officer in the format of the 10 April 2019 Personnel Department memorandum, and current-year.

Divyang / PwBD (Persons with Benchmark Disabilities) Certificate

Assam DME-constituted Medical Board (state quota); a designated MCC centre (AIQ)

What it is
For the Persons with Benchmark Disabilities (Divyang) horizontal quota, 5% of seats in the Assam state pool, applied as per the Rights of Persons with Disabilities Act, 2016 and the MCI notification dated 4 February 2019.
How to get it
For the state quota, a Medical Board assesses your disability at counselling. The DME forms this board from at least three Heads of Orthopaedics departments (or representatives not below Professor) of Assam medical colleges. For AIQ you need a separate certificate from one of the MCC's designated centres.
What it costs
Free at a government facility.
How long
Apply early; assessment is done at counselling.

Watch out: Divyang is not an extra seat on top of other categories: a disabled candidate is adjusted within his/her own SC/ST/OBC/MOBC/General category. A state PwBD assessment does not cover your AIQ PwBD claim, and the other way round.

Char Areas Certificate (CA) — Annexure-VII

Designated Assam authority (Annexure-VII proforma)

What it is
For the Char-area quota: sons/daughters of socially, economically and educationally backward classes who normally live in areas covered by the Assam State Char Areas Development Authority.
How to get it
Get the certificate in the gazette's Annexure-VII format from the designated authority and submit it with your application form.
What it costs
Nominal.
How long
Varies; apply early.

Watch out: This quota is only for residents of the notified Char areas, and the certificate has to be on the Annexure-VII proforma.

Ex-servicemen / Serving Defence Personnel Certificate (ExS/SDP) — Annexure-VIII

As per Ministry of Defence, Government of India norms

Standard
What it is
For children of ex-servicemen and serving defence personnel who hail from Assam.
How to get it
Put together the service/ex-servicemen records and the Annexure-VIII certificate. Selection follows the Ministry of Defence norms (No. 6(1)/2017/D (Res-II) dated 21 May 2018, as amended).
What it costs
Free (service-issued).
How long
Varies; request service records early.

Watch out: Eligibility follows the current Ministry of Defence norms, so check the exact category wording before you rely on the claim.

Freedom Fighter Certificate (FF)

Principal Private Secretary to the Chief Minister of Assam, or the Home Department; plus the concerned DC for the relationship

Standard
What it is
For sons/daughters/brothers/sisters/grandsons/granddaughters of freedom fighters of the State.
How to get it
Get the Freedom Fighter's certificate from the Principal Private Secretary to the Chief Minister of Assam, or from the Home Department issued by an officer not below the rank of Joint Secretary / Deputy Secretary. You also need a certificate from the concerned Deputy Commissioner that establishes your direct relationship with the freedom fighter.
What it costs

How long
Varies; start early.

Watch out: You need both the freedom-fighter certificate and the DC's relationship certificate; one alone is not enough.

Sports Quota proof

Recognised sports bodies / Directorate of Sports & Youth Welfare, Assam

Standard
What it is
For the sports quota (1 MBBS seat). You must qualify in NEET and have represented the state in National/Asian/Olympic games.
How to get it
Put together proof of medals/participation. Merit is ranked by event tier (Olympic/World, Commonwealth/Asian, SAF/National, State and school games). You must have won a medal at least at State level in the three years before the entrance exam.
What it costs
Nominal.
How long
Gather records early.

Watch out: A State-level medal in the preceding three years is the minimum; ties are settled on entrance-test marks, then PCB marks.

The Assam checklist

  • PRC: your main residence proof; apply early.
  • EWS: must be on Annexure-XI, Circle-Officer authenticated, current-year.
  • Category: current OBC/MOBC certificate, freshly dated.
  • Central vs state for AIQ: Assam category certificates are for the state quota; AIQ needs a central-list OBC-NCL certificate.
