Gujarat NEET MBBS / BDS Counselling Documents
Every document a Gujarat candidate needs, covering the national papers everyone must carry plus the ACPUGMEC state-quota proofs. For each one: what it is, how to get it, what it costs, and the mistakes that cost a seat.
Gujarat checks where you were born and the date on your SEBC certificate
Two things decide a Gujarat file. The first is your birthplace: if your School Leaving Certificate does not show you were born in Gujarat, you need a domicile certificate from a Gujarat authority. The second, for SEBC candidates, is that the Non-Creamy Layer certificate must be on the 'Parishishta-4' form from a Gujarat authority and freshly dated for the current cycle. The issue-date rule is strict, and getting it wrong is the most common reason an SEBC claim is rejected.
The two Gujarat traps
SEBC Non-Creamy Layer dating. The 'Parishishta-4' NCL must come from a Gujarat competent authority and be issued within the window the brochure sets for this cycle. An out-of-date certificate drops you to the Open category, so confirm the exact 2026 dates in the prospectus.
Born outside Gujarat? If your School Leaving / Transfer Certificate does not record a Gujarat birthplace, you need a Gujarat domicile certificate.
You are considered for the 85% state quota and the 15% All India Quota run by the MCC. The documents below cover both. One difference for AIQ: your OBC certificate must be on the Central OBC-NCL list in the central format. A Gujarat SEBC / category certificate works for the state quota but is rejected for AIQ. If you plan to use both, get a central-list OBC-NCL certificate as well.

Documents every candidate needs

NEET UG 2026 Admit Card
National Testing Agency (NTA)
- What it is
- This is your proof that you sat the exam. It carries your roll number and the photo used to match your face at the desk.
- How to get it
- Download it from neet.nta.nic.in. A fresh admit card is being issued for the 21 June 2026 re-exam, so your old 3 May admit card no longer counts.

- Free.
- How long
- You can download it the moment it is released, expected by mid-June 2026.
NEET UG 2026 Scorecard / Rank Letter
National Testing Agency (NTA)
- What it is
- This shows your percentile, your marks and your All India Rank. The whole allotment runs on this rank.
- How to get it
- Download it from neet.nta.nic.in once results are out, expected July 2026.
- What it costs
- Free.
- How long
- You can download it the moment results are declared.
Class 10 Certificate / Marksheet
Recognised national or state board
- What it is
- This is the accepted proof of your date of birth. The counselling authority wants the board certificate, not a municipal birth certificate.
- How to get it
- You already have this one. If you have lost it, apply to your board for a duplicate well before counselling starts.
- What it costs
- Free if you have it; a duplicate from the board costs about Rs.100–500.
- How long
- Ready straight away if you have it; a board duplicate can take 2–4 weeks.
Class 12 Marksheet + Passing Certificate
Recognised national or state board
- What it is
- This proves you cleared 10+2 with Physics, Chemistry, Biology/Biotech and English, and met the required aggregate (50% Open/EWS, 40% SC/ST/OBC, 40% Open-PwBD).
- How to get it
- You already have this one. It must show your individual subject marks.
- What it costs
- Free if you have it.
- How long
- Ready straight away if you have it.
Government Photo ID
Aadhaar / PAN / Passport / Voter ID / Driving Licence
- What it is
- This checks your identity against the admit card so nobody can sit in your place.
- How to get it
- Bring any valid, unexpired government ID. Aadhaar is the simplest choice, since your name has to match across all your documents.
- What it costs
- Free if you have it.
- How long
- Ready straight away if you have it.
Passport-size Photographs
Same photo as your NTA registration
- What it is
- These are for the college's records. They must be the same photo you uploaded during NEET registration.
- How to get it
- Print 8 colour copies from the exact file you uploaded to NTA.
- What it costs
- About Rs.50–150 at a photo studio.
- How long
- Same day.
Medical Fitness Certificate
Registered allopathic practitioner (MBBS or higher)
- What it is
- This confirms you are fit for clinical training. Some colleges issue it when you report; others want it in hand beforehand.
- How to get it
- Get it from a registered MBBS doctor, or from the allotted college's own medical board if they insist on using theirs.
- What it costs
- Free at a government hospital; Rs.200–1,000 at a private one.
- How long
- Same day.
Birth certificate: only needed as a backup if your Class 10 certificate does not print the exact date of birth.
Residence & birthplace
These show that you qualify for Gujarat's 85% quota.
School Leaving / Transfer Certificate
Last institution attended
- What it is
- Your academic record and the main proof of where you were born.
- How to get it
- Use the SLC/TC from your last school. Check whether it records your place of birth.
- What it costs
- Nominal / free.
- How long
- Same week if you already have it.
Watch out: If it does not show a Gujarat birthplace, you also need the domicile certificate below.
Gujarat Domicile Certificate
Mamlatdar / Executive Magistrate / Police Commissioner
- What it is
- Needed if you were born outside Gujarat but are claiming the state quota.
- How to get it
- Apply through the Digital Gujarat portal or the issuing office (Mamlatdar / Executive Magistrate / Police Commissioner).
- What it costs
- Nominal (around Rs.20–50).
- How long
- About 2–3 weeks, so apply early.
Watch out: It must come from a Gujarat authority; a domicile from another state is not valid here.
Reservation & fee documents
Only if it applies to youOnly needed if you are claiming a reserved category or a fee concession.
Caste Certificate (SC / ST / SEBC)
Gujarat competent authority
- What it is
- Proof of your reserved category for the Gujarat quota.
- How to get it
- Apply through the Digital Gujarat portal; it must be issued by a Gujarat State authority.
- What it costs
- Nominal.
- How long
- About 2–3 weeks if you don't already have it.
Watch out: A caste certificate from another state is not valid for the Gujarat quota.
Non-Creamy Layer Certificate (Parishishta-4)
Gujarat competent authority
- What it is
- Required for SEBC candidates as proof of non-creamy-layer status, on the prescribed 'Parishishta-4' form.
- How to get it
- Apply through the Digital Gujarat portal in the Parishishta-4 format.
- What it costs
- Nominal.
- How long
- About 2–3 weeks.
Watch out: It must come from a Gujarat authority and be issued within the brochure's date window for this cycle, so confirm the exact 2026 issue dates before you rely on it.

Local Candidate Certificate
Dean of the respective college
- What it is
- Needed for the local-quota seats at NHL Municipal Medical College, Ahmedabad and SMIMER, Surat.
- How to get it
- Get a certificate that you are a local Ahmedabad / Surat city student, signed by the Dean of that institution.
- What it costs
- Nominal.
- How long
- Apply early through the college.
Watch out: Only the Dean's certificate qualifies you for these municipal-college local seats.
EWS / PwBD Certificate
Gujarat authority / MCC centre (AIQ)
- What it is
- EWS for the 10% quota; PwBD for the disability horizontal quota.
- How to get it
- Get EWS through the Digital Gujarat portal for the current year. For PwBD, use the state route (UDID card) for the state quota and a separate MCC-centre certificate for AIQ.
- What it costs
- Nominal; PwBD is free at a government facility.
- How long
- EWS about 2–3 weeks; apply for PwBD early.
Watch out: EWS expires with the financial year. A state PwBD certificate does not cover your AIQ PwBD claim.
Confirming your seat: originals, deposit & PIN fee
ACPUGMEC counselling is run online by the Admission Committee for MBBS and BDS together. To register, you pay a non-refundable PIN fee and a refundable security deposit. To take an allotted seat, you must report, pay the tuition fee and hand over your original certificates at a Help Center to collect the Admission Order. Miss any of these and you lose your security deposit.
Non-refundable Registration PIN Fee + Refundable Security Deposit
Admission Committee (ACPUGMEC), medadmgujarat.org
- What it is
- The two payments that switch on your counselling registration: a non-refundable registration PIN fee plus a refundable security deposit, both set by the Admission Committee.
- How to get it
- Register online at medadmgujarat.org and pay the non-refundable registration PIN fee and the refundable security deposit; then get your documents verified at a Help Center. If you lose the deposit (by not confirming an allotted seat) and want a later round, you have to register again and pay a fresh PIN fee and security deposit.
- What it costs
- PIN fee (non-refundable) + security deposit (refundable); confirm the exact 2026 amounts in the ACPUGMEC information brochure.
- How long
- Pay before the registration deadline the Committee announces for each round.
Watch out: You get the security deposit back only if you exit cleanly within the rules. If you are allotted a seat in Round 2/3/4 and do not confirm it (no tuition fee paid and no originals submitted at the Help Center), you lose the deposit and must register and pay again for the next round.
Original Certificates for Help Center Verification & Admission Order
Admission Committee (ACPUGMEC) Help Center
- What it is
- The set of original certificates you hand over at the Help Center to confirm an allotted seat and collect your Admission Order.
- How to get it
- Once a seat is allotted, report at the Help Center, pay the allotted college's tuition fee and submit your original certificates; only then is the Admission Order issued and your admission confirmed.
- What it costs
- Tuition fee of the allotted college (paid at confirmation).
- How long
- Within the reporting window set for that round; there is no second chance the same day.
Watch out: Document verification at the Help Center is required at registration, and confirming a seat means paying the tuition fee AND submitting originals to collect the Admission Order. If you have not both paid and submitted originals, your admission is not confirmed and you lose your refundable security deposit.
The Gujarat checklist
- Birthplace: check your School Leaving Certificate; if you were not born in Gujarat, get a Gujarat domicile certificate.
- SEBC: Parishishta-4 NCL from a Gujarat authority, freshly dated; confirm the exact 2026 issue-date window.
- Municipal-college local seats: Dean's local candidate certificate for NHLMMC / SMIMER.
- EWS: current financial year only.
- Central vs state for AIQ: Gujarat category certificates are for the state quota; AIQ needs a central-list OBC-NCL certificate.
