Gujarat NEET MBBS Counselling Documents
Every document a Gujarat candidate needs, covering the national papers every applicant must carry, plus the ACPUGMEC state-quota proofs. What each one is, how to get it, what it costs, and the mistakes that cost a seat.
Gujarat watches your birthplace and the date on your SEBC certificate
Two things decide a Gujarat file. First, your birthplace: if your School Leaving Certificate does not show you were born in Gujarat, you need a domicile certificate from a Gujarat authority. Second, for SEBC candidates, the Non-Creamy Layer certificate must be the 'Parishishta-4' form from a Gujarat authority and freshly dated for the current cycle, because the exact issue-date rule is strict and is the most common reason an SEBC claim is thrown out.
The two Gujarat traps
SEBC Non-Creamy Layer dating. The 'Parishishta-4' NCL must be from a Gujarat competent authority and issued within the window the brochure sets for this cycle. An out-of-date certificate drops you to the Open category. Confirm the exact 2026 dates in the prospectus.
Born outside Gujarat? If your School Leaving / Transfer Certificate does not record a Gujarat birthplace, you must produce a Gujarat domicile certificate.
You are considered for the 85% state quota and the 15% All India Quota run by the MCC. The documents below cover both. One difference for AIQ: your OBC certificate must be on the Central OBC-NCL list in the central format. A Gujarat SEBC / category certificate works for the state quota but is rejected for AIQ. If you plan to use both, get a central-list OBC-NCL certificate as well.

National Testing Agency (NTA)

NEET UG 2026 Scorecard / Rank Letter
National Testing Agency (NTA)
- What it is
- Your percentile, marks and All India Rank, the number the whole allotment runs on.
- How to get it
- Download from neet.nta.nic.in after results (expected July 2026).
- What it costs
- Free.
- How long
- Instant download once results are declared.
Class 10 Certificate / Marksheet
Recognised national or state board
- What it is
- The accepted statutory proof of date of birth. The board certificate, not a municipal birth certificate, is what the counselling authority wants.
- How to get it
- You already have it. If lost, apply to your board for a duplicate well before counselling.
- What it costs
- Free if you have it; duplicate ~Rs.100–500 from the board.
- How long
- Instant if held; a board duplicate can take 2–4 weeks.
Class 12 Marksheet + Passing Certificate
Recognised national or state board
- What it is
- Proof you cleared 10+2 with Physics, Chemistry, Biology/Biotech and English, and met the aggregate (50% Open/EWS, 40% SC/ST/OBC, 45% Open-PwBD).
- How to get it
- You already have it. Must show individual subject marks.
- What it costs
- Free if held.
- How long
- Instant if held.
Government Photo ID
Aadhaar / PAN / Passport / Voter ID / Driving Licence
- What it is
- Identity check against the admit card to stop impersonation.
- How to get it
- Use any valid, unexpired government ID. Aadhaar is simplest as the name must match across documents.
- What it costs
- Free if held.
- How long
- Instant if held.
Passport-size Photographs
Same photo as your NTA registration
- What it is
- For college records. Must be identical to the photograph uploaded during NEET registration.
- How to get it
- Get 8–12 colour copies printed from the exact file you uploaded to NTA.
- What it costs
- ~Rs.50–150 at a photo studio.
- How long
- Same day.
Medical Fitness Certificate
Registered allopathic practitioner (MBBS or higher)
- What it is
- Confirms you are fit for clinical training. Some colleges issue it at reporting; others want it in advance.
- How to get it
- Get it from a registered MBBS doctor, or at the allotted college's own medical board if they insist on theirs.
- What it costs
- Free at a government hospital; Rs.200–1,000 privately.
- How long
- Same day.
Birth certificate: only needed as a backup if your Class 10 certificate does not print the exact date of birth.
Residence & birthplace
These establish your Gujarat eligibility for the 85% quota.
School Leaving / Transfer Certificate
Last institution attended
- What it is
- Academic tracking and the primary proof of birthplace.
- How to get it
- Use the SLC/TC from your last school. Check whether it records your place of birth.
- What it costs
- Nominal / free.
- How long
- Same week if held.
Watch out: If it does not show a Gujarat birthplace, you must add the domicile certificate below.
Gujarat Domicile Certificate
Mamlatdar / Executive Magistrate / Police Commissioner
- What it is
- Required if you were born outside Gujarat but claim the state quota.
- How to get it
- Apply through the Digital Gujarat portal or the issuing office (Mamlatdar / Executive Magistrate / Police Commissioner).
- What it costs
- Nominal (around Rs.20–50).
- How long
- About 2–3 weeks; apply early.
Watch out: Must be issued by a Gujarat authority; another state's domicile is not valid here.
Reservation & fee documents

Caste Certificate (SC / ST / SEBC)
Gujarat competent authority

- Proof of your reserved category for the Gujarat quota.
- How to get it
- Apply via the Digital Gujarat portal; it must be issued by a Gujarat State authority.

- About 2–3 weeks if not already held.
Watch out: A caste certificate from another state is not valid for the Gujarat quota.
Non-Creamy Layer Certificate (Parishishta-4)

- Mandatory for SEBC candidates as proof of non-creamy-layer status in the prescribed 'Parishishta-4' form.
- How to get it
- Apply via the Digital Gujarat portal in the Parishishta-4 format.

Local Candidate Certificate
Dean of the respective college
- What it is
- Required for the local-quota seats of NHL Municipal Medical College, Ahmedabad and SMIMER, Surat.
- How to get it
- Obtain a certificate that you are a local Ahmedabad / Surat city student, signed by the Dean of that institution.
- What it costs
- Nominal.
- How long
- Apply early through the college.
Watch out: Only the Dean's certificate qualifies for these municipal-college local seats.
EWS / PwBD Certificate
Gujarat authority / MCC centre (AIQ)
- What it is
- EWS for the 10% quota; PwBD for the disability horizontal quota.
- How to get it
- EWS via the Digital Gujarat portal for the current year. For PwBD, use the state route (UDID card) for the state quota and a separate MCC-centre certificate for AIQ.
- What it costs
- Nominal; PwBD free at a government facility.
- How long
- EWS about 2–3 weeks; apply for PwBD early.
Watch out: EWS expires with the financial year. A state PwBD certificate does not cover your AIQ PwBD claim.
The Gujarat checklist
- Birthplace: check your School Leaving Certificate; if not born in Gujarat, get a Gujarat domicile certificate.
- SEBC: Parishishta-4 NCL from a Gujarat authority, freshly dated; confirm the exact 2026 issue-date window.
- Municipal-college local seats: Dean's local candidate certificate for NHLMMC / SMIMER.
- EWS: current financial year only.
- Central vs state for AIQ: Gujarat category certificates are for the state quota; AIQ needs a central-list OBC-NCL certificate.
