Punjab NEET MBBS Counselling Documents
Every document you need as a Punjab candidate. It covers the national papers every applicant carries, plus the BFUHS state-quota proofs. For each one you get what it is, how to get it, what it costs, and the mistakes that lose a seat.
Punjab checks that you actually went to senior school in the state
Along with a Punjab residence/domicile certificate, BFUHS wants proof that you studied both Class 11 and Class 12 in Punjab. It asks for the Class 11 record on purpose, so that candidates cannot move in just for Class 12. If you took a break after Class 12 you need a gap-year affidavit, and the NRI route goes through a BFUHS eligibility certificate, not the MCC.
What Punjab looks for
Bonafide residence and senior schooling. You need a Punjab Bonafide Resident Certificate in the prescribed Punjab Govt proforma, plus study of 10+1 and 10+2 in Punjab. The Class 11 record is required to show you did not move in only for Class 12.
Punjab-only category certificates. Every reserved or micro-reserved category certificate (except J&K migrants) has to come from the Competent Authority of the State of Punjab; an out-of-state certificate is rejected. Minority candidates (Sikh or Christian) are verified at a designated institute before they can fill minority choices.
You are considered for the 85% state quota and the 15% All India Quota run by the MCC. The documents below cover both. One difference for AIQ: your OBC certificate must be on the Central OBC-NCL list in the central format. A Punjab category certificate works for the state quota but is rejected for AIQ. If you plan to use both, get a central-list OBC-NCL certificate as well.

NEET UG 2026 Admit Card
National Testing Agency (NTA)
- What it is
- This is your proof that you sat the exam. It carries your roll number and the photo used to match your face at the desk.
- How to get it
- Download it from neet.nta.nic.in. A fresh admit card is being issued for the 21 June 2026 re-exam, so your old 3 May admit card no longer counts.

- You can download it the moment it is released, expected by mid-June 2026.
NEET UG 2026 Scorecard / Rank Letter
National Testing Agency (NTA)
- What it is
- This shows your percentile, your marks and your All India Rank. The whole allotment runs on this rank.
- How to get it
- Download it from neet.nta.nic.in once results are out, expected July 2026.
- What it costs
- Free.
- How long
- You can download it the moment results are declared.
Class 10 Certificate / Marksheet
Recognised national or state board
- What it is
- This is the accepted proof of your date of birth. The counselling authority wants the board certificate, not a municipal birth certificate.
- How to get it
- You already have this one. If you have lost it, apply to your board for a duplicate well before counselling starts.
- What it costs
- Free if you have it; a duplicate from the board costs about Rs.100–500.
- How long
- Ready straight away if you have it; a board duplicate can take 2–4 weeks.
Class 12 Marksheet + Passing Certificate
Recognised national or state board
- What it is
- This proves you cleared 10+2 with Physics, Chemistry, Biology/Biotech and English, and met the required aggregate (50% Open/EWS, 40% SC/ST/OBC, 40% Open-PwBD).
- How to get it
- You already have this one. It must show your individual subject marks.
- What it costs
- Free if you have it.
- How long
- Ready straight away if you have it.
Government Photo ID
Aadhaar / PAN / Passport / Voter ID / Driving Licence
- What it is
- This checks your identity against the admit card so nobody can sit in your place.
- How to get it
- Bring any valid, unexpired government ID. Aadhaar is the simplest choice, since your name has to match across all your documents.
- What it costs
- Free if you have it.
- How long
- Ready straight away if you have it.
Passport-size Photographs
Same photo as your NTA registration
- What it is
- These are for the college's records. They must be the same photo you uploaded during NEET registration.
- How to get it
- Print 8 colour copies from the exact file you uploaded to NTA.
- What it costs
- About Rs.50–150 at a photo studio.
- How long
- Same day.
Medical Fitness Certificate
Registered allopathic practitioner (MBBS or higher)
- What it is
- This confirms you are fit for clinical training. Some colleges issue it when you report; others want it in hand beforehand.
- How to get it
- Get it from a registered MBBS doctor, or from the allotted college's own medical board if they insist on using theirs.
- What it costs
- Free at a government hospital; Rs.200–1,000 at a private one.
- How long
- Same day.
Birth certificate: only needed as a backup if your Class 10 certificate does not print the exact date of birth.
Punjab eligibility
This unlocks the 85% state quota.
Punjab Bonafide Resident Certificate
DC / ADC(R) / ADC(D) / SDM / Assistant Commissioner General / DORG / DRO / Executive Magistrate / Tehsildar, or the Commissioner of the Municipal Corporation of Amritsar, Jalandhar or Ludhiana
- What it is
- Your main proof of Punjab residence, and the document that unlocks the 85% state quota. It is issued in the prescribed Punjab Govt proforma to a bonafide resident of Punjab.
- How to get it
- Apply through the Punjab e-services / Sewa Kendra to one of the listed revenue authorities. The prospectus residence proforma (Annexure-II) certifies that you were born in Punjab. If you are eligible through schooling instead, you use the Principal/Headmaster certificate of the Punjab government or recognised school or college you attended.

- Allow about 2 weeks; apply early.
Watch out: It has to be a Punjab certificate in the Punjab Govt prescribed format from the competent authority. The bonafide-resident proforma (Annexure-II) is compulsory for every candidate.
Class 11 Marksheet (+ Class 12 in Punjab)
Respective school
- What it is
- Proof that you studied senior secondary continuously in Punjab, meaning Class 11 as well as Class 12 in the state.
- How to get it
- Use your Class 11 and Class 12 marksheets from Punjab schools.
- What it costs
- Free if held.
- How long
- Instant if held.
Watch out: Punjab asks for the Class 11 record on purpose, to keep out candidates who moved in only for Class 12.
Reservation, gap & fee documents

Scheduled Caste Certificate
DM / ADM / Collector / Deputy Commissioner / ADC / Deputy Collector / First Class Stipendiary Magistrate / City Magistrate / SDM / Executive Magistrate / Extra Assistant Commissioner, or a Revenue Officer not below the rank of Tehsildar (Competent Authority of Punjab only)
- What it is
- Proof of Scheduled Caste status for the Punjab quota, recognised under the Constitution (Scheduled Castes) Order, 1950 in the Punjab Govt format.
- How to get it
- Apply through Punjab e-services to one of the listed competent authorities in the prescribed Punjab proforma. The certificate is issued per the Punjab Welfare Dept letter dated 5.12.1996.
- What it costs
- Nominal (the prospectus sets a reduced application fee for SC candidates).
- How long
- About 2 weeks.
Watch out: Reserved or micro-reserved category certificates (except J&K migrants) have to come from the Competent Authority of the State of Punjab only; an out-of-state SC certificate is rejected. SC reserved-category certificates are verified when you report at the allotted college.
Gap-Year Affidavit
Notary (non-judicial stamp paper)
- What it is
- Needed if you have a break in your education after Class 12.
- How to get it
- Get a notarised affidavit that explains the gap in your education.
- What it costs
- Stamp-paper / notary cost.
- How long
- Same day.
Watch out: Without it, a gap after Class 12 can hold up your verification.
Backward Class (BC) Certificate
Competent Authority of Punjab (Govt of Punjab, Dept of Welfare of SCs & BCs format)
- What it is
- Proof that your community is recognised as a Backward Class under the Punjab Govt notification, certifying that you are not in the creamy layer.
- How to get it
- Apply through Punjab e-services in the prescribed Punjab BC proforma. The certificate has to state that you do not belong to the creamy-layer sections in the Punjab Welfare Dept notification dated 17.01.1994 (as amended).

- Nominal.
- How long
- About 2 weeks.
Watch out: It has to come from the Competent Authority of the State of Punjab only; an out-of-state BC certificate is rejected. The certificate carries a non-creamy-layer declaration in the Punjab format, and BC/SC certificates are verified when you report at the allotted college.
EWS Income & Asset Certificate
Competent Authority of the Govt of Punjab only
- What it is
- For the EWS quota: an Income & Asset Certificate that certifies your family's gross annual income is below Rs.8 lakh and that the family is not covered by the prescribed asset limits. It is valid for the year.
- How to get it
- Apply to the Competent Authority of Govt of Punjab in the prescribed EWS proforma, valid for the relevant year.
- What it costs
- Nominal.
- How long
- About 2 weeks.
Watch out: EWS candidates have to produce a certificate from the Competent Authority of Govt of Punjab only. The asset bars are 5 acres+ agricultural land, a 1000 sq ft+ residential flat, a 100 sq yd+ plot in a notified municipality, or a 200 sq yd+ plot elsewhere; and you must not be SC/ST/OBC (Central List). It is tied to the year, so get one freshly dated for this cycle.
NRI & minority quotas
Only if it applies to youOnly if you are claiming one of these.
BFUHS NRI Eligibility / Equivalency Certificate
Baba Farid University of Health Sciences (BFUHS), Faridkot
- What it is
- Required for the NRI quota: a provisional eligibility/equivalency certificate from BFUHS (not the MCC). NRI candidates also submit a separate physical NRI application form.
- How to get it
- Get the provisional eligibility / equivalency certificate from BFUHS, Faridkot, qualify NEET-UG, then apply on the prescribed NRI proforma on www.bfuhs.ac.in with the prescribed NRI-quota fee, and submit the physical application by the stated last date.
- What it costs
- A prescribed non-refundable NRI fee set by BFUHS (confirm the current amount).
- How long
- Apply well ahead of counselling; the NRI physical application has its own earlier deadline.
Watch out: The NRI application form and the eligibility-certificate form are separate from the regular admission form. Arrange the BFUHS eligibility certificate early; NRI payments have to meet FEMA rules.
Sikh / Christian Minority Documents
Verified at SGRD Institute of Medical Sciences & Research, Amritsar (Sikh) or Christian Medical College, Ludhiana (Christian)
- What it is
- Documents that back a claim under the Sikh-minority or Christian-minority quota. Your eligibility is verified at a designated institute before choice-filling.
- How to get it
- Carry all original certificates plus the document that supports the minority claim (as per the rules or notification of the institution) and report to the designated venue: SGRD, Amritsar for Sikh-minority or CMC, Ludhiana for Christian-minority, during the verification window in the schedule.
- What it costs
- Nominal.
- How long
- Verification runs on the dates set in the prospectus schedule; gather your documents well ahead.
Watch out: If you applied under the Sikh / Christian minority quota you MUST report to the respective institute for verification. Without it, you cannot fill the choices / preferences for those minority-quota seats.
Special & micro-reserved quotas
Only if it applies to youPWD (Persons With Disability) Certificate
A designated centre notified by DGHS, New Delhi (RPWD Act 2016, 21 benchmark disabilities)
- What it is
- For the Persons With Disability quota (category code 16), for the 21 benchmark disabilities under the RPWD Act, 2016.
- How to get it
- Go to one of the designated centres informed by DGHS, New Delhi and get the disability certificate in its prescribed format. Show it to the Principal when you report at the provisionally allotted college.
- What it costs
- Free at a government facility.
- How long
- Apply early.
Watch out: There is no separate medical board for this category and no other disability certificate is accepted; only a certificate from a DGHS-designated centre counts. (For the All India Quota you need a separate MCC-centre certificate.)
Sports Gradation Certificate
Director, Sports, Punjab (merit / eligibility determined by the Dept of Sports, Govt of Punjab)
- What it is
- For the Sports-person quota (category code 17): a sports gradation certificate that grades your sporting achievement.
- How to get it
- Get the Sports Gradation Certificate from the Director, Sports, Punjab. Submit the printed / hard copy of the online-submitted form by hand at the University office within the stipulated period, along with the gradation certificate and self-attested supporting documents.
- What it costs
- Nominal.
- How long
- Apply early; the hard-copy submission has its own early deadline.
Watch out: Your merit and eligibility under the Sports category are decided by the Dept of Sports, Govt of Punjab as per the Punjab Govt notification. The hard copy has to be delivered in person by the deadline.
Backward Area / Border Area Certificate
DC / GA to DC / SDM / Tehsildar of the concerned district (Border Area also needs a Principal/Headmaster study certificate)
- What it is
- For the Backward Area (code 14) or Border Area (code 15) quota: proof that you are a bonafide resident of an area declared backward, or that you come from a town/village within 10 miles of the international border.
- How to get it
- Backward Area: a certificate from the DC / GA to DC / SDM / Tehsildar that you and your family have lived in a declared backward area. Border Area: two certificates, one from the DC / GA to DC / SDM / Tehsildar confirming residence within the 10-mile belt, and one from the Headmaster / Principal confirming at least five years of study in a recognised institution in that town/village.

- Nominal.
- How long
- Varies; arrange before reporting.
Watch out: A certificate from any authority other than DC / GA to DC / SDM / Tehsildar is not valid and will not be accepted. For Border Area you have to both come from within the 10-mile belt and have studied at least five years there (Nursery/LKG/UKG not counted).
Defence (Armed Forces) Ward Certificate
Army / Navy / Air Force Headquarters or the Commanding Officer of the Unit (serving), or the District Defence Services Welfare Officer (ex-servicemen), countersigned by the Director, Defence Services Welfare, Punjab
- What it is
- For wards/widows of Armed Forces (Defence) personnel (category codes 22-30), graded by priority (Priority I to IX) according to the parent's service status.
- How to get it
- Serving personnel: get the certificate from the Army/Navy/Air Force HQ or the Commanding Officer of the Unit, countersigned by the Director, Defence Services Welfare, Punjab. Ex-servicemen: the certificate is signed by the concerned District Defence Services Welfare Officer, countersigned by the Director, Defence Services Welfare, Punjab.

- Varies; request service records early.
Watch out: State-quota defence candidates have to be bonafide residents of Punjab. The priority and the exact wording have to match the parent's status; guardians are considered only if neither parent is alive at the relevant time.
Punjab Police / Para-Military Ward Certificate
Inspector General of Police (HQ), Punjab
- What it is
- For wards of Punjab Police, Punjab Armed Police, Punjab Home Guards and Para-Military Forces personnel (category codes 31-33) killed or disabled (50%+) in action, or winners of the President's Police Medal / Police Medal for Gallantry.
- How to get it
- Get the certificate signed by the IG Police (HQ), Punjab, confirming the parent's service and the killed/disabled/gallantry status, and that you are dependent on them.
- What it costs
- Free (service-issued).
- How long
- Varies; request early.
Watch out: Only the IG Police (HQ), Punjab is the issuing authority. Guardians are considered in place of a parent only if neither parent was alive at the relevant time.
Freedom Fighter Dependent Certificate
DC / ADC / GA to DC of the district to which the freedom fighter belongs
- What it is
- For children / grandchildren of freedom fighters (category code 34): a certificate that the named person is a bonafide freedom fighter granted a Punjab Govt freedom-fighter pension or awarded a Tamra Patra.
- How to get it
- Get the certificate from the DC / ADC / GA to DC of the district to which the freedom fighter belongs, stating the pension/Tamra-Patra grant and your relationship.
- What it costs
- Nominal.
- How long
- Varies; arrange before reporting.
Watch out: A certificate from no other authority is accepted. Guardians/grandparents are recorded per the proforma; strike through whichever does not apply.
Terrorist-Affected / Sikh-Riot-Affected Certificate
DC / GA to DC of the respective district
- What it is
- For children / grandchildren of persons killed in terrorist actions in Punjab (codes 18-19) or in Sikh riots outside Punjab (codes 20-21), and for terrorist/riot-affected or displaced persons.
- How to get it
- Get the relevant certificate from the DC / GA to DC of the respective district, certifying that the named parent / grandparent was killed in (or was affected/displaced by) the terrorist action in Punjab or the Sikh riots outside Punjab, and stating your relationship.
- What it costs
- Nominal.
- How long
- Varies; arrange before reporting.
Watch out: A guardian is considered only when neither parent was alive at the relevant time. Use the exact proforma for your sub-category (children vs grandchildren vs affected/displaced).
J&K Migrant Certificate
Office of the Relief & Rehabilitation Commissioner (Migration), Jammu, Government of Jammu & Kashmir
- What it is
- For migrants from Jammu & Kashmir due to terrorist violence (category code 35).
- How to get it
- Get the certificate from the Office of the Relief & Rehabilitation Commissioner (M), Jammu, Government of J&K.
- What it costs
- Nominal.
- How long
- Varies.
Watch out: Only the certificate issued by the J&K Relief & Rehabilitation Commissioner (M), Jammu is accepted. This is the one reserved-category certificate that does not have to be Punjab-issued.
The Punjab checklist
- Eligibility: Punjab residence/domicile plus Class 11 and 12 studied in Punjab.
- Gap year: notarised affidavit if you have a break after Class 12.
- NRI: get the BFUHS eligibility certificate before counselling.
- Central vs state for AIQ: Punjab category certificates are for the state quota; AIQ needs a central-list OBC-NCL certificate.
