Uttar Pradesh NEET MBBS Counselling Documents
Every document an Uttar Pradesh candidate needs. It covers the national papers every applicant must carry, plus the DGME state-quota proofs and the security deposit. For each one you get what it is, how to get it, what it costs, and the mistakes that lose a seat.
Uttar Pradesh asks for a domicile and a large refundable deposit before you can choose colleges
For the 85% state quota you need a UP domicile certificate, or you must have passed both Class 10 and Class 12 from institutions in UP. Without that, you can only apply for private management seats. The step that catches most people out is the money: before you fill choices you pay a refundable security deposit, and the payment receipt itself is one of the documents checked at verification.
Two gates in Uttar Pradesh
Domicile or 10+12 in UP. You need a valid UP domicile certificate, or you must have passed Class 10 and Class 12 from UP institutions, to take the state quota. If you are not domiciled, you can only apply for private management seats.
Security deposit before choice-filling. The registration fee is about Rs.2,000 (non-refundable). The refundable security deposit is about Rs.30,000 for government seats and Rs.2,00,000 for private seats. Keep the payment receipt, because it is checked at verification. (Confirm the 2026 amounts in the bulletin.)
You are considered for the 85% state quota and the 15% All India Quota run by the MCC. The documents below cover both. One difference for AIQ: your OBC certificate must be on the Central OBC-NCL list in the central format. A UP OBC certificate works for the state quota but is rejected for AIQ. If you plan to use both, get a central-list OBC-NCL certificate as well.

NEET UG 2026 Admit Card
National Testing Agency (NTA)

- This is your proof that you sat the exam. It carries your roll number and the photo used to match your face at the desk.

- Download it from neet.nta.nic.in. A fresh admit card is being issued for the 21 June 2026 re-exam, so your old 3 May admit card no longer counts.

- You can download it the moment it is released, expected by mid-June 2026.
NEET UG 2026 Scorecard / Rank Letter
National Testing Agency (NTA)
- What it is
- This shows your percentile, your marks and your All India Rank. The whole allotment runs on this rank.
- How to get it
- Download it from neet.nta.nic.in once results are out, expected July 2026.
- What it costs
- Free.
- How long
- You can download it the moment results are declared.
Class 10 Certificate / Marksheet
Recognised national or state board
- What it is
- This is the accepted proof of your date of birth. The counselling authority wants the board certificate, not a municipal birth certificate.
- How to get it
- You already have this one. If you have lost it, apply to your board for a duplicate well before counselling starts.
- What it costs
- Free if you have it; a duplicate from the board costs about Rs.100–500.
- How long
- Ready straight away if you have it; a board duplicate can take 2–4 weeks.
Class 12 Marksheet + Passing Certificate
Recognised national or state board
- What it is
- This proves you cleared 10+2 with Physics, Chemistry, Biology/Biotech and English, and met the required aggregate (50% Open/EWS, 40% SC/ST/OBC, 40% Open-PwBD).
- How to get it
- You already have this one. It must show your individual subject marks.
- What it costs
- Free if you have it.
- How long
- Ready straight away if you have it.
Government Photo ID
Aadhaar / PAN / Passport / Voter ID / Driving Licence
- What it is
- This checks your identity against the admit card so nobody can sit in your place.
- How to get it
- Bring any valid, unexpired government ID. Aadhaar is the simplest choice, since your name has to match across all your documents.
- What it costs
- Free if you have it.
- How long
- Ready straight away if you have it.
Passport-size Photographs
Same photo as your NTA registration
- What it is
- These are for the college's records. They must be the same photo you uploaded during NEET registration.
- How to get it
- Print 8 colour copies from the exact file you uploaded to NTA.
- What it costs
- About Rs.50–150 at a photo studio.
- How long
- Same day.
Medical Fitness Certificate
Registered allopathic practitioner (MBBS or higher)
- What it is
- This confirms you are fit for clinical training. Some colleges issue it when you report; others want it in hand beforehand.
- How to get it
- Get it from a registered MBBS doctor, or from the allotted college's own medical board if they insist on using theirs.
- What it costs
- Free at a government hospital; Rs.200–1,000 at a private one.
- How long
- Same day.
Birth certificate: only needed as a backup if your Class 10 certificate does not print the exact date of birth.
Uttar Pradesh eligibility
This is what opens up the 85% state quota.
UP Residence (Niwas) Certificate
UP competent revenue authority (Tehsildar / SDM), via e-District
- What it is
- Your general residence (niwas) proof that you are eligible for the 85% state quota. The bulletin's reporting checklist asks for a 'general residence certificate' (samanya niwas pramaan-patra), not a 'domicile' certificate, and it has to be on the UP-government-prescribed format.
- How to get it
- Apply on the UP e-District portal (edistrict.up.gov.in) with proof of residence, or through a Jan Seva Kendra. It must be on the UP-prescribed format and issued by the UP competent authority.
- What it costs
- Nominal (around Rs.15-30 via e-District / Jan Seva Kendra).
- How long
- Allow about 2 weeks; apply early.
Watch out: It must be on the UP-prescribed format from a UP competent authority. Certificates in the format used for central-government appointments are not accepted. Carry the original plus a self-attested photocopy at reporting.
Security Money Payment Receipt
Generated online after payment
- What it is
- Proof that you paid the refundable security deposit, which is what lets you fill choices and lock a seat.
- How to get it
- Pay online during registration on upneet.gov.in and download the receipt.
- What it costs
- About Rs.30,000 (government) / Rs.2,00,000 (private), refundable; plus about Rs.2,000 non-refundable registration fee.
- How long
- Paid at registration, before choice-filling.
Watch out: No receipt means no choice-filling. The refund follows the rules in the bulletin, so read the forfeiture clause before you lock a seat.
Government Photo ID (Aadhaar / DL / PAN / Passport)
Government-issued
- What it is
- Identity proof you need at reporting. The bulletin's reporting checklist names the Aadhaar card, driving licence, PAN card or passport.
- How to get it
- Use any one valid, unexpired government photo ID, and keep the name the same across all your documents.
- What it costs
- Free if you already have it.
- How long
- Instant if you already have it.
Watch out: Bring the original plus a self-attested photocopy. The name should match your NEET registration.
Reservation & fee documents
Only if it applies to youOnly if you are claiming a reserved category.
Caste (SC/ST/OBC) / EWS Certificate
UP competent revenue authority (Tehsildar / SDM / City Magistrate) in the UP-prescribed format
- What it is
- Proof of your reserved category for the UP state quota. UP vertical reservation is SC 21%, ST 2%, OBC 27% and EWS 10%. The EWS certificate is issued by the Tehsildar / Additional Tehsildar / City Magistrate / SDM and certifies that family income is below Rs.8 lakh, with the standard land/property limits.
- How to get it
- Apply on the UP e-District portal, on the UP-prescribed format, from a UP competent authority. An OBC certificate must carry the Non-Creamy-Layer (NCL) declaration.
- What it costs
- Nominal.
- How long
- About 2 weeks.
Watch out: EWS and OBC certificates must be issued on or after 01 April 2025 to be valid this cycle. Every reservation certificate (other than the disability and NCC certificates) must be on the UP-prescribed format issued by a UP competent authority; certificates in the central-government-appointment format are not valid here.

Divyang (PwBD) Disability Certificate
State-specified board (UP quota) / DGHS, MoHFW, New Delhi (AIQ)
- What it is
- For the disability (Divyang) horizontal quota, which is 5% in UP. The disability certificate does not have to be on the UP-prescribed format.
- How to get it
- For the state quota, use the disability-certification route in the DGME UP bulletin with a valid UDID card. For AIQ, the disability certificate is issued by a Medical Board notified by the Director General of Health Services (DGHS), MoHFW, New Delhi.
- What it costs
- Free at a government facility.
- How long
- Apply early; board slots are limited.
Watch out: A state PwBD certificate does not cover your AIQ PwBD claim, and the other way round.
Freedom-fighter Dependent Certificate
District Magistrate (Jiladhikari), UP
- What it is
- For the freedom-fighter dependent horizontal quota (2% in UP, on the bulletin's prescribed Annexure format).
- How to get it
- Get the certificate on the bulletin's prescribed format, signed and sealed by the District Magistrate (Jiladhikari).
- What it costs
- Nominal.
- How long
- Apply early; sign-off at DM level takes time.
Watch out: It must be on the UP-prescribed format; a central-government-format certificate is not accepted.
Ex-servicemen Dependent (Sainik Aashrit) Certificate
District Soldiers' Welfare & Rehabilitation Officer (Zila Sainik Kalyan evam Punarvas Adhikari)
- What it is
- For the ex-servicemen dependent horizontal quota (2% in UP), for the sons and daughters of war-disabled, discharged or martyred ex-servicemen, on the bulletin's prescribed Annexure format.
- How to get it
- Get it from the District Soldiers' Welfare & Rehabilitation Officer (Zila Sainik Kalyan evam Punarvas Adhikari) on the prescribed format.
- What it costs
- Nominal.
- How long
- Apply early.
Watch out: Carry the ex-serviceman's discharge book / service particulars that the certificate refers to.
NCC 'C' Certificate
National Cadet Corps (NCC)
- What it is
- For the NCC horizontal quota (1% in UP), which needs at least an NCC 'C' certificate.
- How to get it
- You already hold it from your NCC unit; keep the original ready.
- What it costs
- Free if you already have it.
- How long
- Instant if you already have it.
Watch out: A lower-grade NCC certificate ('A' or 'B') does not qualify; the bulletin asks for at least the 'C' certificate. The NCC certificate does not have to be on the UP-prescribed format.
Women Candidate (20% horizontal)
No separate certificate
- What it is
- UP reserves 20% horizontally for women candidates. You do not need any special certificate; your standard identity and board documents establish eligibility.
- How to get it
- Nothing extra to get; your NEET registration and ID establish it.
- What it costs
- Free.
- How long
- Not applicable.
Watch out: This is a horizontal (across-category) reservation, so it sits on top of your vertical category.
The Uttar Pradesh checklist
- Eligibility: UP domicile, or Class 10 and 12 passed from UP.
- Money: pay the security deposit (about Rs.30k govt / Rs.2L private) and keep the receipt for verification.
- Category: current caste/EWS certificates, freshly dated.
- Central vs state for AIQ: UP category certificates are for the state quota; AIQ needs a central-list OBC-NCL certificate.
